State officials hope to begin allocating vaccines for homebound individuals according to recent notice sent out from the town of Burrillville. Those not served by a home health agency have been asked to help the state plan for the effort by filling out a survey found here. The survey does not register the individual for a vaccine but rather, is used for planning purpose.
Homebound individuals are asked to provide contact information here. Those living in Burrillville who need assistance using the survey tool can call the Town’s COVID-19 assistance line at (401) 710-7800 and leave a message with contact information if calling at times when the Town staff is not available.
